Wow that is totally fascinating and disturbing at the same time.
I see know at closer look that the point on the piercing that it just screws on but it looks as intended quite shockingly painful is the first thing that ran across my mind.
I don't know what the other object is in the image but the counter point is eloquent.
Quite a photo!
