You sketch the hand with pencil, make sure to get in the little indications of nails and some lines for the creases and wrinkles. Then you take a graphite stick and rub it over the whole page. Take a paper towel and making little circles, start rubbing the while thing over- smudging and smoothing it out so it loses that grainy quality. Then you take the pencil and define the lines in the hand a little more clearly, take the graphite stick and add shadows and basic shading (pay attention to those big spots in the center and on the side of the hand, depending on your light source). When all that's done, you take your eraser (preferably a Kneaded Eraser so you can shape it and use it to pat thing lighter where necessary) and literally erase your highlights where the skin should be more "shiny" due to the lighting. I was surprised it was such a simple and fast process, as long as you get the main sketch part of it done.
