That's an amazing picture. I love the perspective. This choice make the picture somehow funny. And can you see the tounge? I also like it how it gets slowly blury backwards.

She is a great model when she decides to sit still, but most of the time she turns her back to the camera She really hates to be photographed

Shots like this are difficult to pull off... they don't look good without proper focus and really good lighting, both of which you definitely have! The black and white is a nice addition that really finishes off the shot.
